With a careful coordination of services and a range of choices offered by both statutory and voluntary organisations, Outreach aims to help the Over 50s enjoy an independent life in their own home for as long as possible, with dignity, comfort, a sense of security and a connection with the outside world. Outreach acts a link between potential clients and the available services. The Outreach officer will co-ordinate the activities of these agencies to ensure the satisfactory provision of the specific services required by each individual. Here is an example of services obtained for elderly Pepper Pot members: - Provision of a table, bed, wardrobe and mattress in liaison with Notting Hill Housing Furniture Store. - Free labour supplied for redecorating a flat in conjunction with the Volunteer Bureau. - High backed chair and raised toilet seat provided through liaison with Occupational Therapy. - Assistance with selecting and purchasing a new television. - Successfully applied for funds to enable client to visit his birthplace in the Caribbean. - Successfully claimed medical expenses through travel insurance for illness client suffered in the Caribbean. - Liaised with his housing authority to get a kitchen refurbished and cockroach infestation addressed. - Liaised with Age Concern to arrange home bathing service. - Community Alarm System fitted. - Liaised with his GP with regard to getting a home visit. - Help with understanding utility bills and other paperwork. The Outreach Officer acts as a bridge between the Pepper Pot and its various activities and services and those members who are unable to attend the Centre due to illness. The Outreach Officer also visits members who are unwell in hospital or at home. He also makes regular contact with them by telephone. A Befriending Scheme is soon to be set up which employs the use of volunteers who visit housebound or lonely clients on a regular basis in their homes. Who qualifies for the Outreach Service?
- Members who attend the Centre who become unwell, hospitalised or who stop attending for other reasons. - Housebound members who receive Pepper Pot services such as meals at homes. - All clients who are referred to Pepper Pot to be assessed for appropriate services provided by the Centre are initially visited by the Outreach Officer. - Members who stop attending the Centre, who may be at home and “stuck in a rut”, who may need gentle encouragement to socialize and partake in Centre activities. How to enlist with the Outreach Service?- Through referral by Social Services, a GP, family or friends. - Referral from the Pepper Pot. - Ask for Outreach yourself.
